This is the new government portal for Aged care services and supports, information on Community Care Packages, Respite and Residential Aged Care Facilities (RACFs).   From 1/7/2015  the entry point for Aged Care services ie meals on wheels, home care, community transport, Aged Care Packages (levels 1-4).  Can potentially access short term case management if required (perhaps only 4 hours).


Ability Links NSW – is specifically focused on targeting people with disability, their families and carers aged 9-64 years, who do not access traditional disability support services. Anyone can contact their local Linker and there are no eligibility or assessment requirements.
